Disk counts as non-initialized and formatting doesn't work on MacOS High Sierra

I'm using OpenCore. When I opening disk utility, I see that disk is non-initialized. When I trying to initialize the disk, I getting error "Failed to write last block of device". I have tried Sata-Unsupported, didn't work. Also, I use AHCI.

PC Specs:

CPU - i7 870

GPU - GeForce GTX 560 Ti

Storage - HDD 500 GB

RAM - 8 GB 1333 MHz DDR3
