I have tried a thousand times to get my 6970 working in Sierra and I read a lot of old material about it. So, I have decided to share how I get my GPU working in Sierra. It is simple:
1-) Use Clover and create a config.plist with Mac Pro 5,1 as SMBIOS.
2-) In this config.plist, add this bootflags: -v rootless=0 kext-dev-mode=0.
3-) Inject your device ID in config.plist:
a-) Fake id: 0x67181002
b-) AtiFB: Gibba.
c-) Ports: 5 (this is because I have 2 DVI, 1 HDMI and 2 mDP)
d-) Check INJECT ATI in Clover.
That is it. You will get your 6970 working. If you have problems with your computer freezing when you open some apps like Photoshop, QuickTime and etc., search on web how to patch OpenCL.framework file (you can follow the Mavericks guide, it is working in Mavericks, Yosemite, El Capitan and Sierra. I used to do this when I had a r7 250).
I hope that this mini-guide can help someone who have this awesome, but old GPU.
